| AGP-3908 provides basic hospital and outpatient benefits, the Medicare Part A deductible, skilled nursing facility care and foreign travel benefits. It has a $500 deductible for Part B services. This plan is available with or without prescription coverage. Note: Not available in New York. |

| AGP-3936 has similar benefits as AGP-3908, but covers additional benefits such as hearing aids, chiropractic care and annual physicals. The Part B deductible is not covered and copayments apply for certain services. This plan is available with or without prescription coverage. |
